Media Concent Forms:
Please keep the first page for your own record and return to us the second sheet filled out. The first sheet explains in detail, what this form is for. The backside of the first sheet is the revocation form, you are to take this home and if at any point this year you decide to revoke your permission for us to publish your children's image, please sign and return to us. The front side of the second sheet is the Concent Form its self, telling us what forms of media you are allowing us to publish your children in. The back side is the Non-Concent Form, and should only be filled out if you do not want your child published in anything.
